For your convenience, many forms may be found here for residents to use.
Request for Reasonable Accommodations - This form is used when a resident needs the Housing Authority to provide an accommodation to assist with a disability. For example, grab bars in a shower, or a ramp to get a wheelchair into an apartment. Verification is required from a Physician.
Interim Rent Adjustment Form - This form MUST be completed when there is a change in family composition or income and you wish to have your rent lowered. Please note that proof of any change in family composition or income MUST be documented.
Intent to Vacate Form - form MUST be completed and returned to the Housing Authority office at least 30 days prior to the resident's intent to vacate their apartment.
Public Housing Authority Transfer Request Form - form MUST be completed when a resident wishes to move from one apartment to another apartment within the Housing Authority of McDonough County properties. Submitting a request does NOT guarantee the transfer will be approved.
Visitor Request Form - This form MUST be filled out when you are planning on having a guest in your home. Residents may NOT have guests more than 14 days per year total. A resident MUST also notify the Housing Authority if a guest will be staying more than three days.
Community Center Request Form - This form MUST be completed and returned to the office when a resident wants to use the Community Center.
Satellite Dish Installation Form - This form MUST be completed and returned to the office when a resident wishes to have a satellite dish installed at their apartment. Maintenance staff will be notified of the request. Maintenance staff will mark on the ground where the dish may be installed, and tenant will be notified when they are able to schedule installation. Please note that not all apartments are able to receive the necessary signal needed to use satellite services.
Pet Authorization Form - This form MUST be completed and returned to the office PRIOR to getting a pet. There is a $200 pet deposit that MUST accompany the form. Please remember:
- Cats MUST be declawed and spay or neutered. Documentation must be provided.
- Dogs MUST have received a rabies vaccination and be spay or neutered. Documentation is required.
- Dogs CANNOT be more that 25 pounds; cats, 15 pounds; rodents, 5 pounds.
- Pet authorization is NOT needed for small birds, rodents, fish or turtles.
After you have received authorization from the Housing Authority, you will be given a pet sticker for your apartment door. Please refer to the Pet Policy in your move in packet or ask the office for a copy for complete information.
